After calling to make your appointment, be sure to bring all necessary documents below and visit the FlexSpace at the Eastgate Branch:

• Unsigned DS-11 application form (for a child under age 16,
  please call for more info)
• Proof of Citizenship - original birth certificates,
  naturalization papers, etc., and the photocopies
• Proof of Identity - photo I.D. such as Driver’s
• License and the photocopies
• Passport photo
• Check or money order is required for governmental
  fees, payable to U.S. Department of State (library fees
  are separate)
• Library fee ($35) - the library accepts credit card,
  cash, and check as forms of payment

Can't make it? Passport Services are also available at the Harnish Main Library every day! Hours for appointments and availability for walk-ins is limited, but we will do our best to accommodate you, learn more.
